Register Your Child/Children
The club requires every child to be registered. Please ensure that all the information you give is correct, please complete ALL sections including Special Educational Needs, this will benefit our planning and care of your child. If you change address or telephone numbers please inform the club immediately.
Medical Information
Proper medical information should be given on the form such as the following:
- Long term illness (Asthma)
- Recurrent illness (Allergies)
- Food that makes your child ill or hyper-active
- All forms of behavioural difficulties
If your child requires to take medicine you will need to complete a Medical Form. All inhalers, tablets and liquid medicine must be clearly labeled, in date, obtained from your child’s doctor with clear directions for use.
Booking Form
You will now be required to fill in a Term Time Booking Form, showing which sessions your child/children will require each week. Please read the Finance Procedure Document before completing the Booking Form. With the Booking Form you must return the Payment Form, signed and indicating which method of payment you will be using.
Your Account Sheet will show the days you have booked and you will be charged accordingly. All sessions booked will be charged for, if your child/children do not attend on a day you have booked the morning charge will be payable and the afternoon session will be charged at £7.00. If circumstances change and you wish to change the days please talk to your Manager immediately and change the days on your account sheet.
